  • You can earn over $1,000 to help reduce your annual health care costs
  • Web-based activity program that works with your activity tracker and app
  • Simply set up the tracker to sync with your smartphone, wear it daily-and, walk

United Healthcare Motion Materials

Humana Dental PPO

  • No annual maximum
  • Preventative services are covered at 100
  • Enrollees may receive a discount of up to 20% for Orthodontia service
  • Always get a predetermination of benefits before getting extensive work done

Humana Dental PPO Materials

Humana Vision Insurance

  • Examination once every 12months
  • Members may receive 15% off the retail price or 5% off the promotional price for LASIK or PRK
  • Enrollees may receive a 20% discount on items not covered by the plan
  • Always ask your provider if they will match the cost for contact lenses, such as 1-800-Contacts or Warby Parker

Humana Vision Insurance Materials

Voya Accident Insurance (New)

  • Provides better benefits with a lower rate of accidental injuries
  • Same Coverage options for you and your eligible dependents.
  • Guaranteed Issue (No Medical Questions)
  • 24-hour coverage (on or off the job)
  • $50 Annual Health Screening Benefit
  • This plan offers portability which allows you to keep your existing coverage even if your employment status changes

Voya Accident Insurance Materials

Voya Critical Illness Insurance (New)

  • You may elect a $10,000 or $20,000 benefit amount, which will pay you upon diagnosis of any one of the covered critical conditions
  • Your spouse and children are eligible for up to 50% of the amount you elect. Children are automatically included at no additional cost
  • Coverage is guaranteed issue, meaning no medical questions are required at the time of enrollment
  • The plan covers multiple illnesses, so if you’re diagnosed with more than one of the covered critical conditions, you will receive a benefit for each one
  • Recurrence benefit- pays a benefit a second time for which you already received a benefit as long as you are symptom and treatment free for at least 6 months

Voya Critical Illness Insurance Materials

Voya Hospital Indemnity Insurance (New)

  • Pays benefits for hospital admissions and confinement (regular & ICU)
  • This could be for any reason – Sickness, Injury, Pregnancy, COVID, etc.
  • There are no pre-existing condition limitations, even for pregnancy
  • Coverage is guaranteed issue, meaning no medical questions are required at the time of enrollment
  • This plan has a rehabilitation facility benefit in addition to the admission and confinement benefit
  • Lower premium amounts
  • This plan offers portability which allows you to keep your existing coverage even if your employment status changes

Voya Hospital Indemnity Insurance Materials

Short-Term Disability & Long-Term Disability Insurance

Short Term Disability:

  • 100% paid for by Kalera
  • Receive 60% of your income up to $2,000 per week
  • Benefits begin after 7 calendar days and continue for up to 12 weeks

Long Term Disability:

  • 100% paid for by Kalera
  • Receive 60% of your income up to $12,500 per month
  • Benefits begin after Short Term Disability ends and continue until you reach the Social Security retirement age

Short-Term Disability & Long-Term Disability Insurance Materials

Basic & Voluntary Life Insurance

Basic Life:

  • 100% paid for by Kalera
  • Receive 1x your salary up to $350,000 if you participate in the medical plans offered by Kalera
  • Beneficiaries will receive a lump sum payment if you die while employed by Kalera

Accidental Death and Dismemberment (AD&D):

  • 100% paid for by Kalera
  • Receive 1x your salary up to $350,000 if you participate in the medical plans offered by Kalera
  • Provides payment to you or your beneficiaries if you lose a limb or die in an accident

Voluntary Life & AD&D Insurance:

  • Guaranteed coverage up to $100,000 or five times your salary and up to $25,00 for your spouse when you are first eligible
  • Employee: Up to five times your salary in increments of $10,000; $300,000 maximum
  • Spouse: Up to $150,000 in increments of $5,000
  • Children: Up to $10,000 in increments of $10,000

When am I Eligible to Enroll?

Full-Time team members, who work 30 hours per week. Your benefits will be effective the 1st of the month following 60 days of employment.
